The Ministry of Health of Ukraine has approved the form of a certificate of contraindications to vaccination from Covid-19, the press service of Ministry of Healthcare reports.
The department approved the form № 028-1/о. It will be needed by Ukrainians who have been ordered to be vaccinated against coronavirus for work, for example, school staff, but they have contraindications to vaccination. This certificate will be the basis for the employee not to be dismissed without vaccination.
Thus, the form № 028-1/о will be filled in on the basis of medical records about the patient's health.
The Ministry of Health has published instructions on how to fill in this certificate.
It is noted that the certificate of temporary contraindications to vaccination must contain the validity of these contraindications. Persistent contraindications may be to vaccination with a specific vaccine against Covid-19, if there was an allergic reaction to the first dose or one of the components of the drug.
